Extension lag

Saturday, 26 June 2010

In finger injuries, the PIP joint occasionally just won’t actively extend to neutral. It is rarely NOT because of the extensor mechanism. So apart from working the living daylights out of the extensor mechanism with exercises, is there a role for dynamically holding the PIP joint in extension with, say, a capener? Is it valid to think that resting the extensor mechanism in a shortened position will improve the length-tension relationship, and thereby the overall extension of the finger? I can remember some great results - but I can also think of a few non-events... Until I can get some motivation to investigate it properly, my treatment paradigm is: exercises for the intrinsic and extrinsic extensor tendons, maintenance of PIP joint passive extension, use of capener during the day, and a static splint overnight.
